Licensure Verification Request

The Daphne Cockwell School of Nursing will complete forms and provide documentation and verification of clinical placements and curriculum for TMU alumni who are undergoing licensure verification to work in a jurisdiction outside of Ontario. Clinical and curricular breakdowns for studies that occurred prior to 2010 will be based on estimates and the limited documentation on hand.

Due to the volume of requests received, please allow 7-8 weeks for your order to be fulfilled.

We do not provide official transcripts. If you require one as part of your application, please submit a request through MyServiceHub and upload the transcript with your files, or request the transcript be sent to [email protected]. For degree verification, confirmation letters, or other documents not related to licensure please follow the request process on the TMU documents and letters webpage.
